Fitness Training Requirement

Before our fitness professional can develop an individual
exercise program for you, 
we will need for you to complete
a pre-participation or medical screening questionnaire.
This questionnaire will allow us to determine what fitness
or health assessments need to be completed before you
start your exercise program.
